    This is an Alberta Health long-term cancer research project called "Alberta's Tomorrow Project", see https://myhealth.alberta.ca/Alberta/Pages/albertas-tomorrow-project.aspx If you signed up (could've been years ago), you are still on file with them. They check in every few years or so with the study participants.

    This is Alberta's Tomorrow Project, a long term cancer research project (website: https://myatp.ca/). Taken directly from their website: Welcome to Alberta’s Tomorrow Project (ATP), a longitudinal study tracking the health of 55,000 adults in this western Canadian province. Launched in 2000, ATP’s mission is to investigate why some people develop cancer and chronic diseases like heart disease and diabetes, while others do not. The answers to be discovered over the next several decades will not only help shape how cancer and other illnesses are detected, diagnosed, and treated – but most importantly, how they can be prevented in the first place. Albertans between the ages of 35 and 69 years, from all walks of life, and from all regions of the province, stepped forward to participate in ATP, the largest research project in the province’s history.
